I understand that intuitive readings and ThetaHealing sessions are being provided by Sarah Itkin, Intuitive Healer, at my request.
I understand that Intuitive Counselors and ThetaHealing practitioners do not diagnose mental health disorders, medical conditions, prescribe substances, nor interfere with treatment of a licensed medical and/or mental health professional. I understand that Sarah Itkin is not a licensed physician and that intuitive readings and ThetaHealing sessions are complementary to and separate from licensed medical services. Always seek the advice of a qualified healthcare provider if you have any questions regarding a medical or health condition.
I understand that Sarah is not a veterinarian and that information Sarah receives as a pet intuitive should not replace that of a professional animal care provider.
Information provided by Sarah Itkin should not be relied upon as a substitute for advice, programs, or treatment that you would normally receive from a licensed professional, such as a lawyer, doctor, psychiatrist, psychotherapist, veterinarian, or financial adviser.
Although Sarah was a Licensed Clinical Social Worker and former mental health therapist, I understand that intuitive readings and ongoing intuitive counseling sessions are not considered to be psychotherapy or a replacement for psychotherapy. I assume sole responsibility for my own physical and mental health and for the results of any sessions provided by Sarah Itkin that may affect me in any way.
Clients under the age of 18 must have written permission from a parent or guardian. All children under the age of 16 must be accompanied by an adult.
Confidentiality: All sessions are confidential unless you are in immediate danger of hurting yourself or others, or you are committing child/elder abuse.
Cancellation and refund policy for individual sessions and packages:
Unless there is a last-minute emergency, appointments canceled less than 24 hours in advance are considered a missed appointment and I will be charged in full for the service. Sessions are non-refundable after a reading has been completed.
Packages are non-refundable. Appointments canceled less than 24 hours in advance will be considered a missed appointment and will be subtracted from the total number of sessions in the package.
All information I received from Sarah Itkin is accepted with full knowledge that any action I took as a result of the information received is my complete responsibility.
I have read, understood, and accepted the informed consent as shown on Sarah Itkin’s website. I hereby acknowledge and affirm my understanding and agreement with the above.
